Unusual Esophageal Diverticulum in an Elderly Patient

An 83-year-old female patient with a history of ischemic heart disease, arterial hypertension, and functional dependence, was admitted with acute symptoms of melena, nausea, and anorexia. On admission to the emergency department, he was found without signs of shock, with a positive rectal examination for melena and documentation of anemia without transfusion criteria. Esophagogastroduodenoscopy was performed, which reported two cardiac mucosal tears, one of them with signs of recent bleeding, for which clip hemostasis was performed without complications. Additionally, a diverticulum was found in the cervical esophagus, initially classified as Zenker's diverticulum, for which characterization was indicated using upper gastrointestinal radiography (Figure 1).
