Rare Cause of Liver Injury in a Young Patient

We present the case of a 21-year-old female patient, with no relevant history, who consulted for abdominal pain of one year of evolution, associated with afternoon fever and weight loss of 20 kg (29% of total body weight). On physical examination, she presented with cutaneous mucosal pallor and painful hepatomegaly on palpation. Laboratory tests showed a hematocrit of 32%, hemoglobin 10.2 g/dL, leukocytes 16750 /mm3, total bilirubin 6.4 mg/dL (VN 1 mg/dL), direct bilirubin 5.3 mg/dL (VN 0.5 mg/dL) and alkaline phosphatase 228 U/L (VN 109 U/L); the rest of the parameters were normal. Blood cultures, viral serology, and autoantibodies were negative.
