Hidden Mass: Diagnostic Challenge in a Teenager with Acute Abdominal Pain

A 14-year-old female patient with a history of obesity (BMI 43.8) presented to the emergency department with periumbilical abdominal pain associated with nausea and vomiting that had been ongoing for 48 hours. On examination, she presented with a distended, painful abdomen in the umbilical region and epigastrium, without peritonism or palpable visceromegaly. Blood chemistry, serology, stool parasites, and stool cultures were all negative. A multidetector computed tomography (MDCT) scan with intravenous contrast was performed first, followed by magnetic resonance imaging (MRI) for better characterization of the case.
