Cystic Teratoma of Pancreas. Diagnostic Challenge and Therapeutic Management

Abstract

Pancreatic teratoma is an extremely rare benign congenital neoplasm. We present the case of a 61-year-old woman who presented with a 3-month history of abdominal pain. An abdominal ultrasound revealed a hypoechoic nodular lesion in the duodenopancreatic region, with a focal internal echogenic area and well-defined margins. A computed tomography scan demonstrated a 34 × 42 mm hypodense nodular lesion arising from the uncinate process, containing areas of fat density and focal calcification, in close contact with the superior mesenteric vein. Diagnostic evaluation was complemented with magnetic resonance imaging and endoscopic ultrasound-guided fine-needle biopsy, which showed no evidence of malignancy. Laboratory tests revealed normal tumor marker levels. The patient underwent a converted laparoscopic pancreaticoduodenectomy, and histopathological examination demonstrated morphological findings consistent with a mature cystic teratoma.
